Horizon Blue Cross and Blue Shield of New Jersey

Florence Kariuki is a nursing leader at Horizon Blue Cross Blue Shield of New Jersey, serving as Director, New Models and Episodes of Care Programs. She is responsible for crafting the strategy, design, and implementation of Episodes of Care programs and other innovative Alternative Payment Models. Among other initiatives, Ms. Kariuki also led the design and implementation of Horizon’s Enterprise-wide Social Determinants Of Health (SDOH) and Home-Based Palliative Care programs, as well as Population Health initiatives focused on improving transitions of care, optimizing Care Coordination infrastructure, and reducing potentially avoidable hospital utilization. Ms. Kariuki promotes a payer/ provider integration strategy which prioritizes stakeholder collaboration to effectuate consumer and community centric care models, built upon the tenets of the triple aim. She is a task Force member of the Conversations of Your Life (COYL) initiative by the NJHQI, a fellow with the New Jersey Healthcare Executive Leadership Academy (NJ HELA) and serves as a Board Member for the Health Coalition of Passaic County (HCPC), an Alumni Board Member for Seton Hall University, and an Advisory Board Member on the Prometheus Advisory Board. Among other publishing, she is a Co-Author on the 22 8th Edition textbook on Policy & Politics in Nursing and Health Care.
